Why the lines are still there
A fractional laser treats the skin in a grid of tiny dots and leaves untreated skin in between. The preview has to leave the lines there too.
The colour is where this laser wins: sun spots, brown age spots and mottled damage clear, and the tone evens out across the whole face. The lines are a different story. Every wrinkle in the photograph is still in the preview, each one finer and shallower, and the deeper ones change least. That is one pass, which is what the patient is deciding about. Passes can be repeated months apart, and the skin keeps improving toward what a full resurfacing does in one operation. The preview shows the pass in front of them, not the end of a course.

How is this different from the deep CO2 laser resurfacing simulator?
Mostly it is a question of how much, and in how many visits. This simulator shows one pass of a fractional laser: the colour clears and the lines soften without going away. Deep resurfacing is the fully ablative operation, done once, where the wrinkling largely goes and the skin tightens. Fractional passes can be repeated months apart and the result keeps building toward the same place, which is the trade a patient is actually weighing: one longer recovery against several shorter ones. If your practice offers both, the two simulators are separate on your account and a patient previews whichever one you turn on.
Does the preview show one treatment or the finished result?
One pass. That is deliberate, because one pass is what the patient is booking and what they will see in the mirror afterwards. Fractional treatments are usually a course, spaced months apart, and the skin keeps improving with each one. Setting the expectation at a single pass is the part that protects the consult: a patient who expected the whole course from one visit is disappointed at the follow-up, and that conversation lands on your coordinator.
How realistic are the fractional CO2 previews?
The model is fine-tuned with a double board certified plastic surgeon, more than three decades in practice, and tuned separately for men and for women. It is built around the mechanism. A fractional laser treats a grid of tiny dots and leaves skin untreated in between. So the preview softens lines rather than erasing them, and leaves the folds, jowls and jawline exactly where they were.
Does the preview tighten the skin or the eyelids?
No. That belongs to the deeper, fully ablative pass, where the skin contracts and the eyelid skin tightens with it. A fractional treatment does not shrink the skin, so the simulator does not tighten anything: no hanging fold flattens and the jawline and neck are unchanged.
Does the simulator work for every patient’s skin tone?
No, and the widget says so before it renders anything. CO2 lasers can permanently change pigment in darker skin, so the patient is asked to match their skin against a tone scale first. Where the laser isn’t right for their skin, they’re told that and pointed back to your practice rather than shown a result. They still reach you as a lead.
What about acne scarring?
The preview softens it slightly and leaves every scar visible. Fractional lasers are widely marketed for acne scarring, so it is the claim most worth being careful about. The model is deliberately conservative here. A preview that erased the scarring would promise something the treatment does not deliver on its own.
What stops a patient treating the preview as a promise?
The widget tells them on the result screen, in plain words. Previews are visualizations that support the consultation: individual results vary, nothing is guaranteed, and what is achievable for a particular patient’s skin is the surgeon’s determination. The preview’s job is to start that conversation, not to settle it.
