Ageless AI vs PreviewMD · Verified August 2026

The surgical alternative to Ageless AI.

Ageless AI previews non-surgical treatments: botox, filler, GLP-1 weight loss, hormone and peptide therapy, hair restoration. PreviewMD previews surgery, on models fine-tuned with a practicing board certified plastic surgeon’s decades of experience, at $249 to $999 a month with a 14-day trial.

Ageless AI

What is Ageless AI?

Ageless AI is an AI preview platform for aesthetic practices, contracted through Ageless AI LLC with RepeatMD as its billing agent. A patient uploads a selfie and sees photorealistic previews of treatments on their own face or body, alongside a Beauty Score that rates facial symmetry and skin quality against others in their age group, and a Future Aging view of themselves 15 to 20 years out with and without treatment.

The treatment set is non-surgical. Their own pages lead with botox and filler and extend to GLP-1 weight loss, hormone optimization, peptide therapy and hair restoration. Their fee schedule is public: $499, $699 and $999 a month for 50, 100 and 150 patients, with enterprise pricing above that.

What Ageless AI does well

They publish a real price list, which most of this category refuses to do, and they publish it with the awkward parts included: overage rates, a subscription pause fee, annual terms. That is more honest than a demo gate and we would rather say so.

The distribution behind them is the serious part. RepeatMD reported 2,500 client practices at its November 2023 Series A, and Ageless lands inside that footprint rather than starting cold. For an injectables-led practice their procedure coverage is genuinely better than ours, because ours is zero.

Where Ageless AI falls short

Nothing in their treatment set is surgical. There is no facelift and no breast augmentation, so for a surgical practice the overlap with PreviewMD is close to none. This is a lane difference before it is a feature comparison.

The per-patient economics run differently. Their entry plan is $499 for 50 patients, about $10 a patient, with overage at $10 per patient above the cap; ours is $249 for 150 previews, about $1.66 each, with overage at $1.00. Read both fee schedules against your own volume before deciding.

Their master service agreement grants Ageless “a non-exclusive, worldwide, perpetual, irrevocable, fully paid-up, royalty-free right and license” to use practice training photos for model improvement, and customer data sits on their platform until termination. PreviewMD processes a photo and stores nothing, so there is nothing to license. Whether that matters is a question for your own counsel, not a claim from us.

What Ageless AI costs

Both companies publish a price list, so this is a rare comparison in this category where nobody has to book a call to see a number. The gap is per patient, not per month.

First-party

Their fee schedule lists Starter at $499/month for 50 patients, Growth at $699 for 100 and Pro at $999 for 150, with Enterprise quoted above that. Annual billing is 15% off. The first overage month is free, then $10 per patient above the tier cap. A subscription pause costs $99/month. Checked 12 August 2026.

Per patient

At the entry tier that is about $10 a patient before overage. PreviewMD’s entry tier is $249 for 150 previews, about $1.66 each, with overage at $1.00. The right comparison is your own monthly volume against both fee schedules, not the headline price.

Contract

Monthly plans renew month to month; annual plans run a one-year initial term and auto-renew unless either party gives 60 days’ notice. Cancellation requires 24 hours’ notice before the next billing date. PreviewMD is month to month with a 14-day trial and no notice period.

PreviewMD

What is PreviewMD?

PreviewMD is an AI preview widget for surgical results. A patient uploads one front-facing photo on your website and sees a photorealistic, anatomically correct before and after in about three to four minutes. The facelift simulator and the rhinoplasty simulator are among them; the simulators page has the full list.

One script tag installs it on any platform under your branding. Every preview delivers a lead before the patient sees their result, and photos run through the render without being stored.

The widget as a patient meets it on your site. Your branding, your domain, and the privacy answer stated up front. Elite removes the PreviewMD mark entirely.

What PreviewMD does well

The model was fine-tuned with a double board certified plastic surgeon who has practised for over thirty years, separately for men and for women, which is what a surgical preview needs before anyone will trust it.

The price list is public and the trial is self-serve with a card up front, so a practice can be running it the same day without a sales cycle.

Where PreviewMD falls short

Ageless AI covers a treatment set PreviewMD does not touch at all: botox, filler, GLP-1 weight loss, hormone optimization, peptide therapy and hair restoration. If those drive your revenue, we are not a substitute and it would be dishonest to pretend otherwise. They also have distribution we don’t, launching inside RepeatMD’s installed base, and a Future Aging feature with no equivalent in our product.

What PreviewMD costs

$249 per month for Starter with 150 previews included, $449 for Pro with 500, $999 for Elite with 1,200. Extra previews are a flat $1.00 each and the widget never shuts off mid-month. Annual billing is two months free. Every plan starts with a 14-day trial, card up front, cancel anytime. No setup fee, no per-patient fee, no hardware.

Which should your practice choose?

Choose Ageless AI if

Injectables, weight loss and hormone programs are the bulk of your book.
You want a patient to see themselves 15 to 20 years out, which PreviewMD doesn’t do.
You are already inside the RepeatMD ecosystem and want one supplier.

Choose PreviewMD if

Surgical procedures drive your revenue.
You want a preview from a model a board certified plastic surgeon validated, not one described as a screening tool.
You would rather no company held a perpetual license to your before-and-after photos.
Your monthly volume makes $1.00 overage cheaper than $10.

How switching from Ageless AI works

There is little to migrate, because the procedure sets barely overlap. A practice running Ageless for injectables and PreviewMD for surgery is a reasonable outcome rather than a compromise, and nothing in either product prevents it.

If you are replacing rather than adding, the practical steps are the notice period and the photos. Their annual terms auto-renew unless you give 60 days’ notice, and their agreement describes customer data deleted on termination. PreviewMD installs as one script tag and takes nothing with it.

Questions

The questions buyers ask.

Is PreviewMD an Ageless AI alternative?

For surgical practices, yes, and for injectables-led practices, no. Ageless AI previews non-surgical treatments; PreviewMD previews surgery, on models fine-tuned with a practicing board certified plastic surgeon. The procedure sets do not overlap.

How much does Ageless AI cost compared to PreviewMD?

Both publish pricing, which makes this straightforward. Ageless AI’s fee schedule lists $499 a month for 50 patients, $699 for 100 and $999 for 150, with overage at $10 per patient. PreviewMD is $249 a month for 150 previews, $449 for 500 and $999 for 1,200, with overage at $1.00. Check both against your own monthly volume.

Does Ageless AI store patient photos?

Their master service agreement describes customer data held on their platform and deleted on termination, and grants Ageless a perpetual, irrevocable, royalty-free license to use practice training photos for model improvement. PreviewMD processes the photo to generate the preview and stores nothing. Read their agreement yourself and decide what your practice is comfortable with.

Is Ageless AI part of RepeatMD?

Their master service agreement names Ageless AI LLC as the contracting entity with RepeatMD acting as its authorized billing agent, and RepeatMD announced the launch. Treat them as related rather than identical.

Can a practice run both?

Yes, and some should. The procedure sets barely overlap: Ageless AI covers the injectables and wellness book, PreviewMD covers surgery. Nothing in either product blocks the other, and a practice doing both kinds of work has a reasonable case for both.

Does Ageless AI preview a facelift or a breast augmentation?

Not per their own pages as of August 2026. Their treatment set is non-surgical: botox and filler through GLP-1 weight loss, hormone optimization, peptide therapy and hair restoration. Those are the procedures PreviewMD has live.
